Copays
You pay just $10 for exam . . . and $15 for materials (lenses and/or frame).
LASIK
Reduced fees with participating eye doctors. You pay no more than: $1,800 for one eye . . . or$3,600 for both eyes
* If you choose a frame that costs more than the amount the plan allows, you'll simply pay the modest additional charges.
