OHS is pleased to continue to offer the employees of Jackson Memorial Hospital County and Jackson Health
System two plans this year. Both plans feature Diagnostic and Preventive care at no cost and
all other procedures at substantial discounts.
The OHS Plan 200 is your Standard option. It
offers you significant savings on complicated procedures. Our Plan S-400 is the Enriched Plan.
This plan offers you increased savings and the added benefits of specialty dental care coverage
and discounted vision services.

Specialty Care
In making your decision about which plan is best for you, keep in mind one important detail.
The Enriched Plan S-400 is a Specialty plan. This means that if you should require the care of
a specialist, you pay the same amount as you would pay to a general dentist. The co-payments
listed for the Standard Plan 200 only apply in a general dentist's office. If you should need
to see a specialist with this plan, you receive a 25% discount off the specialist's usual and
customary fees.
Family Choice
The Family Choice benefit will allow each member to select his/her own OHS dental facility.
If members have children living in separate households or a spouse who wants to choose a dentist
closer to his/her workplace, this new benefit was designed with them in mind.
At OHS we know that every family has individual needs, and one dentist may not satisfy all
of them. OHS gives members the choice to select the dentist that best satisfies the need of
each individual family member.
Family Care Pedodontist
Primary Care Pedodontist are available for children 10 and under.
Pedodontist are dentists
that treat children. As a member of Jackson Memorial Hospital County and Jackson Health System, you have the
option to select a pedodontist for your child or may choose to have your child or may choose
to have your child see a general dentist.
How You Will Save

Note:
Members enrolled in the Plan 200 may have their dependents assigned to any OHS provider
in the state of Florida. Those enrolled in the Plan S-400 may select a provider from OHS' South
Florida network, in Dade and Broward counties. The enclosed listing of participating dental
providers are available for either plan, the Standard or Enriched option. For additional providers
accepting the standard option, you may obtain a Statewide Provider Directory at an enrollment
session or from your Department Personnel Representative.
Upon request Oral Health Services, Inc. shall provide written information about
the terms and conditions of the plan to prospective enrollees.
Oral Health Services, Inc. is licensed and approved by the State of Florida
Department of Insurance as a prepaid limited health service organization under Chapter
636 of the Florida Statutes.
