How It Works

1.
Your plan brochure tells you about your benefits, and you can choose a network provider at www.compbenefits.com.

2.
Depending on your plan, either you or your doctor will download a VisionPass Form from www.compbenefits.com*. You must use the form in the time specified for services**.

3.
Visit your doctor, who will provide you a comprehensive eye exam and order prescribed eyeglasses or contacts, if necessary.

4.
Pay any copayments. That's the end of your "paperwork."

5.
CompBenefits pays the doctor directly for his or her professional services.

It really is as easy as that!

* If you don't have access to the Internet, just call our Member Services team at 800-865-3676, and they will mail a form to your home.

** If you do not use your form in the time specified for services, you won't be able to download another until the next time you are eligible for benefits. However, you can request an extension from our Member Services team at 800-865-3676.
