
Summary of Benefits - Traditional
Regular
professional dental care is important to maintaining healthy teeth and gums.
With rising dental fees, it can also be quite expensive. Your selection of
the Traditional Dental Program will provide professional dental care while
helping you control dental expenses.
With the
Traditional Program, you have two options when seeking dental treatment. You
may visit a dentist of your choice within the traditional network at 100% /
90% / 50% benefit levels. Or, you may visit any dentist out of the
Traditional network at 100% / 80% / 50% benefits levels.
There is
an annual benefit maximum of $1,500. If you choose a dentist out of network,
there is also a $50 annual deductible for basic and major services. The
program also includes a $1,500 lifetime orthodontic benefit. See your
schedule of benefits for more details. In order to receive benefits, a claim
must be filed with CompBenefits. If you have questions concerning claims

After you
enroll in the Traditional program, you will receive an identification card,
with a maximum of two cards per family. Your member number will be printed
on the front of the card with a toll-free hotline on the reverse side. You
can call the hotline from 8:00 a.m. to 6:00 p.m. if you have any questions.
The benefits
under the Traditional program are arranged and provided by your employer.
The role of CompBenefits is limited to providing only those
administrative services Emory has contracted with CompBenefits to
provide, including certain claim services and administration of network of
contracted care providers. The program is not underwritten or insured by CompBenefits.
If you
enroll in dental coverage, you must remain in the program selected for a
period of 12 months.
The directory
contains a list of the contracted general and specialty dentists. This
network is offered through and administered by CompBenefits.
Choice Of
Dentists
You are free to select any Traditional participating dentist or Traditional
participating specialist for any appointment you schedule. The Traditional
Program participating dentists are listed alphabetically by town or city.
The complete address and phone numbers are shown for each office listed. If
you wish, you may select a different personal dentist for each covered
dependent so that each covered dependent can receive dental care where it is
most convenient.
Making An Appointment With Your Dentist
When scheduling an appointment, be sure to identify yourself as an Emory
Traditional Program member. This will ensure that you receive the full
benefits available to you. When you arrive for your appointment, it will be
necessary to show the dental office your Traditional Program ID card.
Specialist Care
The Traditional Program provides a network of specialist dentists. Specialty
dentists in each city are listed alphabetically by the specialty types
below:

There is no
need for a referral to see a specialist in the Traditional Program
network.
