What is Plan One?
Plan One is a DHMO Plan also known as the C250ZPX
plan. This plan requires you to select and be assigned to a participating general dentist provider. The Humana/CompBenefits Provider Directory provides a listing of participating general dentists. These dentists are contracted with Humana/CompBenefits to charge you according to the C250ZPX
schedule of benefits and copayments.
How do I obtain a listing of Participating Providers?
You can obtain a listing of participating DHMO Providers including General Dentists and Specialists (Endodontist, Orthodontist, Oral Surgeon, Pediatric Dentist, Periodontist, and Prosthodontist) through this website at Plan One - Search for Providers or by contacting our Customer Services Department at 800-594-0977.
How do I select or change my assigned Participating General Dentist Provider?
You may select or change your assigned participating general dentist provider simply by contacting our Customer Services Department at 800-594-0977. You may also register through this website at MyCompBenefits.com. Once you have registered at MyCompBenefits.com, you would then follow the instructions for selecting your provider by clicking on the policy number and then clicking on the provider search to start the process. Any provider selections processed by the 15th of the month
will be effective the first of the following month.
How many times a year can I visit my Participating General Dentist Provider?
You are encouraged to visit your dentist regularly. With your Humana/CompBenefits dental plan, you are not limited to a specific number of visits per year.
How can I get more information?
You can contact our Customer Services Department at 800-594-0977, M-F, 8am-6pm EST. Customer Services can assist you with changing your dentist, information about your plan, or obtaining emergency services.
Is there any maximum coverage limitation?
There are no limitations on benefits.
How do I make appointments?
Making an appointment is easy. Simply call the participating general dentist provider office you have selected to schedule an appointment. Please contact the participating provider on or after the date you receive your certificate of coverage and ID card. Your enrollment information with that participating provider will already be on hand, confirming that you are eligible for treatment with their office.
What if I need a Specialty Dentist?
Should you need a Specialty Dentist (i.e., Endodontist, Oral Surgeon, Periodontist, Prosthodontist, Pediatric Dentist), you may be referred by your Participating General Dentist, or you may refer yourself to any Participating Specialty Dentist. Upon identifying yourself as a Humana/CompBenefits member, you will receive a 25% reduction from usual and customary fees for services performed. Specialty services are available only in areas where the dental plan has a Participating Specialty Dentist.
What if I need an Orthodontist?
Should you need an Orthodontist, you may be referred by your Participating General Dentist, or you may refer yourself to any Participating Orthodontist. Upon identifying yourself as a Humana/CompBenefits member, you will be charged according to the fixed co-payments outlined in Schedule of Benefits. See your Schedule of Benefits and Co-payments for specific amounts. Orthodontic coverage is available for both Adults and Children.
How do I pay for services?
If your visit is for covered preventive care, like a routine exam, cleaning, or x-ray, there is no charge for the procedure. The Humana/CompBenefits program prepays the dentist. For other procedures, a copayment may be required. See your Schedule of Benefits and Copayments for specific amounts. You pay the copayments directly to the participating provider.
What if I go to a non-participating dentist?
You will not be eligible for any benefits with non-participating dentists. You must receive treatment from the participating general dentist provider you have selected.
