HOW THE PLAN WORKS
Your dental plan is designed to help you and your family get
the dental care you need by offering preventive care and restorative dental
treatments. Your plan covers cleanings, fluoride, exams, oral hygiene
instruction, and x-rays every six months. The plan covers, as necessary,
space maintainers, tooth extraction, root canals, treatment for gum disease,
fillings and crowns. The complete list of covered dental care services are
listed in the Schedule of Benefits. Procedures not listed on the Schedule of
Benefits that are performed by your selected participating dentist may be
charged at that provider's usual and customary fee. You can ask your
participating dentist for a benefit determination and cost estimate before
you receive any dental treatment. To find out what is covered, refer to your
Schedule of Benefits.
This Handbook contains a description of covered services and
benefits as well as copayments, limitations and exclusions. There is a
helpful glossary located in the appendix of this handbook that gives
definitions of words found in this Handbook that may be unfamiliar related
to dental services, administration and benefits offered by the plan.
