DEFINITIONS

Annual Benefit Maximum No more than $1,000 in benefit will be paid on an annual
basis for the period July 1st through June 30th of the following year, per Member.

Benefits are those Dental Care Services available to the Members as stated in their Handbooks.

Dental Care Services are those services to be performed by a Participating General Dentist's or Participating Specialty Dentist's office where Members shall receive Dental Care Services.

Dental Facility is the office of a Participating General Dentist or Participating Specialty Dentist where Members shall receive Dental Care Services.

Effective Date is the first day that a Member is entitled to receive Benefits designated in the Handbook.

Handbook and Evidence of Coverage (hereinafter referred to as "Handbook") is that document provided to the Member that specifies Benefits and conditions of Coverage.

Member is a covered eligible enrollee.

Necessary Treatment are required Dental Care Services determined by the Participating Dentist to establish and maintain Member's health and appearance.

Participating General Dentist and Participating Specialty Dentist (hereinafter
referred to as "General Dentist" and "Specialist") are those licensed dentists selected and contracted with Company as independent contractors to provide dental Benefits to Members.

Schedule of Benefits are those services covered under the Healthy Kids dental plan.

Treatment Plan is the Participating Dentist's recommended Dental Care Services for the Member. The Member may request a written copy.

Usual Charges are those fees that are customarily charged for Dental Care Services by the Participating Dentist. Company does not determine said charges.
