MEMBER RIGHTS & RESPONSIBILITIES
We are dedicated to establishing a relationship with you
that is based on mutual respect and fairness.
The following member rights and responsibilities form the
foundation upon which CompBenefits, its providers and members can build such
a relationship.
Your Rights
-
You have the right to be treated with respect. You have the right to privacy and to have your dignity
recognized.

You have the right to be assisted promptly and courteously
when you seek advice and assistance.

You have the right to receive information about your
dental treatment in terms you can understand.
-
You have the right to receive information about
CompBenefits, its benefit plans, its services and its network of
providers. You have the right to receive information about your
rights and responsibilities as a plan member.

You or your designated representative has the right to
voice complaints or appeals about the plan or the care you receive. You have the right to request information as to how to file
a complaint or request an appeal about a complaint resolution.

You have the right to select a participating general
dentist, to change that participating general dentist, and to know who the dentists are who provide your
dental care.

You have the right to an honest discussion of appropriate
and necessary dental treatment options for your condition, regardless of cost or benefit coverage.

You have the right to take part in making decisions with
your dentist about your dental care. You have the right to ask your dentist for information in order
to make a sound decision regarding a dental procedure.

You have the right to refuse treatment. If you refuse
treatment, you have the right to ask your dentist about the result of refusing.

You have the right to the confidentiality of your dental
records and all other information unless you allow it to be released, or unless the law requires it to be
released.

You have the right to ask your dentist for appropriate
guidance and recommendations for additional dental care when you are no longer eligible for
coverage.
Your Responsibilities
-
You have the responsibility to be considerate and
respectful of your dentist and their staff.

You have the responsibility to cooperate with your
dentist. It you have questions or disagree with the treatment plan, you have the responsibility to discuss
it with your dentist.

You have the responsibility to keep all scheduled
appointments and to be on time. If unable to keep an appointment, notify your dentist as early as possible.
-
You have the responsibility to provide, as best you can,
information that the dental plan and its practitioners need to know to care for you.
-
You have the responsibility to follow the treatment plan
and instructions for care you have agreed to with your dentist.
-
You have the responsibility to understand the medications
prescribed by your dentist and whether you are scheduled for follow-up visits.
-
You have the responsibility to be considerate of other
plan members and to be patient if delays are encountered.
-
You have the responsibility to pay for all services not
covered under the plan and for all services that exceed the annual benefit maximum.
-
You have the responsibility to know what services are
covered under your dental plan. If you have questions about what your
dental plan covers, refer to this Handbook, your Schedule of Benefits, or
call Member Services at 1-877-531-3026.

You have the responsibility to express opinions, concerns
or complaints regarding your dental care and to do so in an appropriate
manner.

You have the responsibility to present your ID card before
you receive services. You have the responsibility to pay all charges, if
any, for missed appointments and non-covered services.

You have the responsibility to read the information
provided to you by CompBenefits about your dental plan.
